Wedding Questionnaire

The following is a list of questions that will help us perform your wedding ceremony. Please take a moment to review the questions, and write down the answers with your future partner and families. Together we will create the perfect wedding ceremony for your needs.

1. What is the exact Date, Place, and Time?

2. What are the full names of the Bride and Groom?

3. Will someone be giving the Bride away? If so, who?

4. Will there be an exchange of rings, or only one ring, or no rings at all? Are there any other rituals that you would like to have?

5. Do you want a Civil ceremony, or a Religious one? If Religious, what are the Faith backgrounds of your two families? Are there any specific spiritual principles that you would like the ceremony to express?

6. Will you be writing your own vows? Do you want Ted to read a special poem or scripture passage? Will you have any special music played?

7. Is there anything else you can think of, that you'd like us to know?


Marriage License - You can get this the same day you apply, but you must both appear before the Clerk of the Court, with ID. Arizona does not charge military personnel to mail them the certified copy of the license; you need only pay $72 for the license itself. Non-military must pay $26.50 for the certified copy to be mailed. They should explain all your options to you at the Clerk's office. For information, call (602) 506-3360 or 506-2125.
